Post your nomination on this Facebook page and include the person’s or project’s name along with a brief description of how they have made a difference.
In November, they’ll conduct a poll on Facebook to determine the winning person and project. The winners will be announced in December, featured on their page with short videos of their stories, and recognized by County Supervisor Dianne Jacob at the Live Well, San Diego! Building Better Health in East County Leadership Team meeting in January.
